3. PMDP MMC Form C ā€“ Agency Screening Certification Form (To be accomplished by the Office Personnel / Administrative Officer)

4. PMDP Form D ā€“ Declaration of Medical IllnessesĀ (To be accomplished by the Nominee)

5. PMDP Form E ā€“ Physician Certification with Laboratory Results attached (To be accomplished by a Physician not related to the nominee, from a Government or Private Hospital or the Nomineeā€™s Agency Clinic/Hospital, and taken within the last 6 months at the time of application)

6. Updated Personal Data Sheet ā€“ CSC Form 212 (To be accomplished by the nominee)

7. Certified True Copy of CSC Form 33 (Appointment Papers) – (To be accomplished by the Office Personnel / Administrative Officer)

8. Certified True Copy of Designation Order (if applicable) –(To be accomplished by the Office Personnel / Administrative Officer)
9. Certified True Copy ofĀ Transcript of Records of all Educational Attainments

10. Certified True Copy of PSA-issued Birth Certificate

11. Copy of the Organizational Chart

12. Copy of the IPCR for the past two (2) years with Adjectival Rating

13. Certificate of No Pending Administrative / Criminal Case

14. Nominee Certification Form- To be accomplished by Nominee

15. PMDP Letter of ConformeĀ – To be accomplished by theĀ Nominee and the Head of the Agency / Nominating Officer, (to be submitted once accepted to the Program only)
